To be eligible to participate in the Food Distribution Program, the household must contain at least one person who is a member of a federally recognized tribe.

Eligible households are certified based upon income and resource standards set by the federal government and must be recertified at least every 12 months.

The following information is needed to determine your eligibility for the Food Distribution Program.

MEMBERSHIP CARD OR CERTIFICATE OF DEGREE OF INDIAN BLOOD (CDIB)
 
VERIFICATION OF IDENTITY       
Drivers license, work or school identification cards, Social Security cards, CDIB cards, or Birth Certificates, medical identification cards.     

UPDATED VERIFICATION OF INCOME
A copy of checks, pay stubs (totaling one month), letter of awards on Social Security, VA, unemployment, pensions, etc. showing the gross amount of income.
If anyone in the household has no income and is 18 years or older, you will need to provide verification of their unemployment status.  Two statements signed by two adults, not related to you or the unemployed person.
 
VERIFICATION OF RESIDENCY
Our service area consists of Osage County.  A current utility bill with your name and address on it is required.
